Two nurses graduate from the same college, pass the same NCK exam, and start work the same month. Three years later, one earns barely KES 40,000 a month at a rural dispensary while the other clears six figures in a Nairobi private hospital or a Gulf contract. Same qualification, wildly different pay.
Nurse salary in Kenya is not one number — it is a moving target shaped by who employs you, where you work, your job grade, your specialisation, and a stack of allowances most students never think to ask about.
Here is the uncomfortable truth we will unpack below: the single decision that changes your pay the most is not how hard you study, but which employer and cadre you enter — and there is one recent legal ruling that quietly reset what interns and new nurses can earn. Let us break it all down.

A Kenyan nurse's monthly pay is built from two parts: a basic salary tied to a job grade, and a set of allowances on top. The allowances often make the difference between an unlivable and a comfortable salary — which is exactly why they dominate every nurses' strike.
In the public service, nurses are graded on a scheme of service. Registered Nurses historically ran from Job Group H at entry up to Job Group P, while the degree-holding Nursing Officer cadre starts higher and rises to director level (Kenyans.co.ke). Since a 2017 job evaluation, the Salaries and Remuneration Commission (SRC) has been migrating these old job groups into new Common Salary Grades (The Standard).
One warning before any number: there is no single public SRC table mapping each nurse cadre to an exact basic salary. The widely shared "Registered Nurse III earns KES X" tables come from secondary sites and disagree with each other, so treat every job-group figure in circulation — including the ones below — as indicative, not gospel.
What a New Nurse Actually Earns
The most reliable evidence of entry pay is not a blog table — it is an actual county recruitment advert. A Uasin Gishu County advert for Registered Nurse III (KRCHN diploma holders) set the basic salary scale at KES 26,900 – 35,380 per month, plus a house allowance of KES 4,500 and commuter allowance of KES 4,000 (Uasin Gishu County Public Service Board). A Narok County advert for Enrolled Nurse III started around KES 25,420 (Narok County Government).
So a realistic public-sector entry basic for a new KRCHN is roughly KES 27,000–35,000, before allowances. Once house, commuter and other allowances are added, gross take-home rises — which is why market aggregators quote higher "KES 50,000+" figures. The lesson: always separate basic from gross, because unions negotiate over basic while your bank sees gross.

The Intern Stipend: Where Pay Got Complicated
Before you reach a permanent salary, most graduates go through internship — and this is where Kenyan nurse pay became a national controversy.
In a directive dated 13 March 2024, the SRC advised the Ministry of Health to cut the stipend for a degree-holding nursing intern from around KES 105,000 (the old Job Group K rate) to between KES 35,000 and KES 50,000 (Employment and Labour Relations Court judgment, Kenya Law). Under the same directive, medical officer interns were set at KES 47,000–70,000, and diploma clinical officer interns at KES 27,000–35,000 (Capital FM).
Unions were furious. The doctors' union called it a roughly 91% cut from the previous KES 206,000 intern package, while the Kenya National Union of Nurses (KNUN) rejected the proposal outright (The Standard). The "KES 20,000" figure many nurses quoted was net take-home after deductions, not the SRC's gross band.
The dispute went to court — and on 17 October 2024 the Employment and Labour Relations Court upheld the SRC directive, ruling it was justified on budgetary grounds and dismissing the petitions (Kenya Law). For a new nurse, the takeaway is blunt: your internship stipend is now materially lower than it was for the cohort just ahead of you, and it is legally settled.

This is the "wait, that's in my payslip?" section. In the landmark 2017 CBA between KNUN and the Council of Governors, nurses were promised a stack of enhanced allowances, including a uniform allowance of KES 50,000 a year, a health-risk allowance raised to KES 5,000/month, an extraneous allowance, a non-practising allowance of KES 6,000–20,000, and a commuter allowance of KES 15,000–25,000 (The Standard).
But here is the catch every nurse should know: that 2017 package was largely never implemented across the counties — only a handful, such as Mombasa, Machakos and Migori, actually paid the enhanced allowances, and the uniform figure many nurses continued to receive was closer to KES 10,000 a year (Business Today). This gap between what was signed and what was paid is the single biggest reason nurses keep striking — including in 2026.
The allowances that reliably move your take-home are:
- House allowance — set by SRC housing "clusters": Cluster 1 is Nairobi (highest), Cluster 2 covers Mombasa, Kisumu, Nakuru and major municipalities, and Cluster 3 is everywhere else (lowest) (Tuko).
- Commuter allowance — commonly KES 3,000–25,000/month depending on grade (Tuko).
- Health-risk, extraneous, uniform and emergency-call allowances — which vary by county and are frequently the subject of disputes.
Because house and commuter allowances scale with your grade and town, two nurses on the same basic salary can take home noticeably different amounts.
Employer Changes Everything
If one factor dominates your pay, it is your employer. The same nurse can earn very differently across these settings:
- County government (public): most nurses work here. Pay follows the public scale plus allowances, but salaries can be delayed (see the next section). Entry basic around KES 27,000–35,000 (Uasin Gishu CPSB).
- National referral hospitals (e.g. Kenyatta National Hospital): market data puts an average registered nurse there around KES 32,000/month basic, rising with seniority and allowances (Payscale — market estimate/Salary/30335f13/Kenyatta-National-Hospital)).
- Large private hospitals (Aga Khan, Nairobi Hospital): salary aggregators indicate registered nurses commonly earn KES 55,000–100,000/month, the higher end of the local market (Glassdoor — market estimate).
- Faith-based and NGO facilities (e.g. CHAK/mission hospitals): pay tends to track near or slightly below the public scale and is donor-dependent, though no single published rate exists (CHAK).
- Abroad: this is where the numbers jump. Kenyan nurses recruited to Saudi Arabia have been offered a minimum of around KES 150,000/month for fresh graduates, with accommodation and other benefits on top (Daily Nation). In the UK, a newly qualified staff nurse sits on NHS Band 5, roughly £29,969–£36,483 a year for 2025/26 (NHS pay band data).

Role and Specialisation: How Pay Grows
Within any employer, your pay climbs with seniority and specialisation. In the public service the ladder runs from Registered Nurse III at entry up through Senior, Chief and Principal grades, with the degree-holding Nursing Officer cadre topping out at Director of Nursing Services — a jump of several multiples in basic pay from bottom to top (Kenyans.co.ke).
Specialisation pays. Market data (indicative, not official) suggests:
- Critical care / ICU nurses command around KES 100,000–130,000/month, well above general ward pay (market estimate).
- Theatre, renal, oncology and anaesthetic nurses similarly earn premiums for their Higher Diploma qualifications.

Why Kenyan Nurses Aren't Always Paid on Time
A salary you are promised is not the same as a salary you receive on the 28th. When Kenya devolved health to the 47 county governments at the end of 2013, pay and industrial relations fractured — the country has seen more than 20 health-sector strikes since, including a nurses' strike that ran 150 days in 2017 (BMJ Global Health / PMC).
Delayed salaries remain a live problem: counties have been reported holding around KES 44 billion in unpaid worker dues built up over three years, with Nairobi alone accounting for the bulk (Daily Nation).
In mid-2026, two big pay stories are running at once:
- A nationwide strike. KNUN issued a strike notice dated 22 July 2026 and downed tools on 29 July 2026, citing grievances dating back to the 2017 CBA and demanding conclusion of the 2025–2029 CBA (People Daily). The Employment and Labour Relations Court in Mombasa later suspended the strike and ordered nurses back to work pending determination (Daily Nation).
- UHC absorption. The government confirmed that Universal Health Coverage staff — including thousands of nurses on insecure short-term contracts — would move to permanent and pensionable terms from 1 July 2026, backed by about KES 8.6 billion approved by the National Assembly, with 7,414 workers verified for transition (The Star).
