Pharmacology is the unit that most Kenyan nursing candidates underestimate — until it costs them the exam. It appears in both Paper I and Paper II, it tests calculation accuracy under time pressure, and the NCK frequently uses it to separate candidates who understand from candidates who memorised. Why Pharmacology Costs Candidates the Most Marks

Pharmacology content is distributed across both NCK papers for all cadres. For KRCHN candidates, pharmacology constitutes approximately 20–25% of Paper I based on publicly reported candidate patterns, with drug calculation and mechanism questions appearing consistently. For BScN candidates, advanced drug interactions, polypharmacy, and antidote management extend the pharmacology load into Paper I and occasionally into Paper II's evidence-based practice questions.

The brutal reality: a candidate who drops 5 pharmacology marks per paper loses 10 marks total. That is often the difference between passing and resitting.

Mistake 1: Confusing Drug Mechanisms With Drug Indications

NCK MCQs rarely ask "what is the mechanism of metformin?" They ask it indirectly: "A patient with Type 2 Diabetes Mellitus is started on metformin. Which of the following effects would the nurse expect?" If you only know the indication (lowers blood glucose) and not the mechanism (reduces hepatic glucose output, increases peripheral insulin sensitivity per Rena et al., 2017, Diabetologia, DOI:10.1007/s00125-017-4342-z), you will pick the wrong distractor.

How to fix it: For every drug class you study, always learn three things together:

Mistake 2: Confusing Antidotes

Antidote questions are high-yield in NCK pharmacology because they test specific knowledge that cannot be guessed. The Kenya Ministry of Health Poisoning Management Guidelines define the following critical pairings that every candidate must know cold:

Mistake 3: Drug Calculation Errors — The Wrong Formula

This is the category mentioned at the start: dosage calculation errors caused by applying the wrong formula to the wrong question type. NCK uses three core calculation question types, and each requires a specific approach.

Type 1: Tablet/Oral Dose

Formula: Dose required ÷ Dose available × Quantity of tablet or liquid

Example: Prescriber orders Amoxicillin 500 mg. Stock is 250 mg per capsule. How many capsules?

> 500 ÷ 250 × 1 = 2 capsules

Type 2: IV Infusion Rate (drops per minute)

Formula: Volume (mL) × Drop factor ÷ Time (minutes)

Example: 1,000 mL of Normal Saline to infuse over 8 hours. Drop factor: 15 drops/mL. What is the rate in drops per minute?

> 1000 × 15 ÷ (8 × 60) = 15,000 ÷ 480 = 31.25 ≈ 31 drops/minute

Type 3: Weight-Based Paediatric Dosing

Formula: Dose (mg/kg) × Weight (kg) = Total dose required

Example: Paracetamol is prescribed at 15 mg/kg for a child weighing 20 kg. What is the dose?

> 15 × 20 = 300 mg

The common fatal error: Candidates mix up drop factor values. The standard drop factor for a standard giving set is 15 drops/mL. For a paediatric micro-drip giving set, it is 60 drops/mL. Applying the wrong drop factor halves or quadruples your answer. Learn this distinction before your exam.

Mistake 4: Missing Drug Contraindications

NCK questions frequently test contraindications in a clinical scenario format: "A nurse is about to administer X. Which patient condition should prompt the nurse to withhold the drug and report to the physician?"

The most frequently tested contraindication pairs in recent NCK cycles, per candidate-reported question patterns:

  • ACE inhibitors (e.g., Captopril, Enalapril) → Pregnancy (teratogenic, especially 2nd and 3rd trimester, per WHO Model Formulary)

Mistake 5: Getting Routes of Administration Wrong

NCK pharmacology questions frequently include the route of administration as a distractor. The correct route is sometimes the entire point of the question — not the drug itself. Critical routes to know precisely:

  • Magnesium Sulphate (MgSO₄) for eclampsia: Administered IV (slow push) or IM (deep IM, gluteal), never subcutaneously. The Kenyan MOH BEmONC protocol specifies the loading dose as 4g IV over 20 minutes, followed by 1g/hour maintenance.
  • Adrenaline (Epinephrine) for anaphylaxis: The first-line route is IM — anterolateral thigh, not IV (IV is only for cardiac arrest). This is per KNHA/WHO guidelines.
  • Oxytocin for PPH prevention: Given IM or slow IV — never as a fast IV bolus, which causes dangerous cardiovascular effects per MOH Kenya EmONC guidelines.
  • Insulin: Subcutaneous — not IM, not IV (except Actrapid/Regular insulin in DKA management, which is IV infusion).

Mistake 6: Drug Interactions — The High-Yield Pairs

Drug interaction questions test whether the nurse can anticipate harm before administering a drug. The pairs most commonly tested in NCK pharmacology:

  • Warfarin + Aspirin/NSAIDs → Increased bleeding risk (additive anticoagulant + antiplatelet effect)
  • ACE inhibitors + Potassium-sparing diuretics (e.g., Spironolactone) → Hyperkalaemia
  • MAO inhibitors + Tyramine-rich foods / SSRIs → Serotonin syndrome (a severe, life-threatening reaction)

  • Gentamicin + Frusemide → Compounded ototoxicity and nephrotoxicity

The exam pattern: The question presents a patient on Drug A who is prescribed Drug B. The nurse must identify the interaction risk and respond appropriately — usually by withholding, monitoring, or reporting.

Mistake 7: Ignoring the Kenya Essential Medicines List (KEML)

The Kenya Essential Medicines List (KEML) is the MOH-approved formulary governing which drugs are available in Kenya's public health facilities at each level of care. NCK questions use KEML-approved drug names and dosages. Generic names, not brand names, are used throughout.

Candidates trained in private institutions or using international textbooks sometimes encounter different drug names or doses. NCK will always use the Kenyan standard.

Examples where KEML differs from international texts:

  • Kenya uses frusemide (not furosemide — same drug, different spelling convention)
  • Adrenaline (not epinephrine) is the preferred NCK terminology
  • Pethidine is still listed for obstetric pain in some Kenya protocols (morphine preferred internationally)

Week 1: Antidotes, contraindications, drug interactions (all the high-specificity items — no calculation required, pure memorisation with understanding)
